At Northern Virginia Hand&Nerve Center, we aim to make your visit as smooth and comfortable as possible. We are committed to providing the highest quality care and ensuring that you feel prepared and informed every step of the way.
Before Your Visit
Once your appointment is scheduled, you will receive a confirmation email and text with your appointment details.
Complete the following patient forms before your first visit. Download them in advance to save time upon arrival.
If you can't fill out the forms yourself, a friend or family member can help. You just need to sign once they're done.

What to Bring

Current driver license or state identification
This is required to verify your identity. The clinic needs to ensure that the patient being treated is indeed the person on file, which helps prevent medical identity theft. Bring a physical copy of your driver's license or state-issued ID.
Referral letter, if applicable
If your primary care doctor or another specialist has referred you to this clinic, the referral letter provides context for your visit. It includes important details about your medical history and the reason for the referral. Bring a printed copy of the referral letter or have it sent directly from your referring doctor.
Current insurance cards
Your insurance information is necessary to process claims and determine coverage for the visit. It helps the clinic understand your benefits and any out-of-pocket costs you might incur. Bring your physical insurance card(s) so the office can make a copy for their records.
List of current medical conditions, medications and supplements
This is required to verify your identity. The clinic needs to ensure that the patient being treated is indeed the person on file, which helps prevent medical identity theft. Bring a physical copy of your driver's license or state-issued ID.
Copies of any lab results
Previous test results provide the clinic with important diagnostic information, reducing the need for repeat tests and enabling a quicker understanding of your condition. Bring printed copies or digital files (on a USB drive or emailed in advance) of any relevant lab results, x-rays, MRIs, or other diagnostic tests.

During Your visit

Check-in
Upon arrival, our front desk staff will welcome you and assist you with the check-in process. If you haven’t already done so, you’ll be able to complete any outstanding forms at this time.

Consulatation
You'll meet your hand surgeon to review your history, discuss symptoms, and conduct an exam. We'll address your concerns and outline your care plan. If needed, we may do imaging like X-rays or ultrasounds to assess your condition. Based on the findings, your surgeon will diagnose and discuss treatment options, whether surgery, therapy, or other treatments, to create a personalized plan for you.

After Your Visit

Before you leave, we’ll ensure that all follow-up appointments are scheduled, and you’ll receive detailed instructions on your treatment plan. Our team is always here to support you, and you’re welcome to reach out with any questions after your visit.
